Our goal is to treat the whole patient to restore vitality, function and quality of life. Dr. Zetoony specializes in the treatment of sleep disorders in adults and children and adults with a variety of pulmonary diseases.

Patient Forms
* If you were referred by another provider's office, please obtain referral if one is required by your insurance.
- Prior testing you have completed, i.e. chest x-rays, EKG, breathing tests or sleep studies will be helpful to bring with you to your first appointment.
- If you are a SLEEP patient using a device (CPAP, oral appliance, etc) for treatment of apnea, please bring with you to your first appointment.

PLEASE NOTE: DO SLEEP SOLUTIONS, INC is not a MEDICAID CONTRACTED PROVIDER. The office does not accept this insurance as PRIMARY or SECONDARY. DO SLEEP is NOT A PROVIDER for TRICARE PRIME. These appointments would be considered SELF PAY. Please note that DUE TO STAFFING BURDEN WE ARE NOT ABLE TO SCHEDULE NEW PATIENTS WITH HMO PLANS AT THIS TIME.
